What are your hours?

At Foundations Therapy, we're open and ready to serve you throughout the week, from Monday to Friday, between 8:30 am and 6:00 pm.

What services do you provide?

We pride ourselves in offering comprehensive therapy services including physical, occupational, speech, and language therapies. But that's not all! We also specialize in dyslexia evaluations with reading remediation, feeding therapy, and sensory integration therapy.

What age group of children does Foundations Therapy cater to?

Our team at Foundations Therapy is trained and experienced in working with children of all ages, from newborns to 21 years old.

How soon can my child start therapy at Foundations Therapy, and what is the evaluation process?

Our goal is to get your child started as soon as possible. Once you reach out to us, our office staff will guide you through our thorough new patient intake process. This includes gathering all necessary information about your child and obtaining a prescription from your primary care physician. We coordinate with your insurance provider to ensure coverage and aim to schedule the evaluation and potential therapy sessions at your earliest convenience.

If my child was evaluated at another facility, do we need to go through the evaluation process again?

Not to worry! At Foundations Therapy, we accept evaluations from other facilities provided they were completed within the last three months and contain all the necessary information for treatment in the Plan of Care.

Does Foundations Therapy accept insurance?

Yes, we do! We're currently in-network with several insurance providers including Blue Cross Blue Shield, United Healthcare (UHC), Tricare, and CMS/Medicaid Title 19 and Title 21.

What if my insurance provider is not in your network?

In such cases, as a courtesy, our staff will assist you in submitting claims as an "out-of-network provider". If insurance coverage is not available, we offer a "private pay discount". For more details, feel free to speak to our office.

Besides insurance, are there other ways to pay for your services?

Absolutely! We accept the Step Up for Students Scholarship and also offer a private pay discount.

Does Foundations Therapy conduct community conferences?

Yes, indeed! We have a history of conducting informative conferences on a variety of topics like Sensory Processing Disorder, Executive Functioning, Language Literacy Development, Infant Massage, and Feeding and Oral Motor Development. These conferences are aimed at parents, educators, and other professionals. If interested, please contact our office for further details.

Insurance submissions are completed in house so that we can provice individualized information about your child. Being a family-run company, it is important to support you through this process and be able to answer questions in “real time.”

Therapy Session FAQ

What is the aim of the evaluation?

The evaluation determines if there is a need for treatment, what the appropriate treatment method is, and identifies goals for the child.

How long is an initial evaluation?

The initial evaluations are scheduled for 1 hour.

What is included in an evaluation?

At Foundations Therapy, our evaluations provide a comprehensive assessment which includes interviewing parents and getting the child’s history followed by standardized testing (when possible) and clinical observations. The results of this comprehensive evaluation identify the child’s strengths, limitations, and functional skills.

What assessment tools are used during the evaluation process?

The “right” or “best” test to administer depends on what reason that the evaluation is being conducted (e.g., different scales may be administered to determine program eligibility vs. treatment planning). Tests administered will vary based on age, stage, and purpose of the evaluation.

What happens after the evaluation?

Following the evaluation, the therapist will score, interpret, and contact the parent/guardian to discuss the evaluation findings. Once the recommendations are discussed with the parent, the therapist will complete the written report and Plan of Care, sign it, and send the POC to the primary physician for doctor’s signature.

What is a Plan of Care (POC)?

The Plan of Care (POC) is a document which provides a summary of the evaluation findings followed by the therapist’s recommendations and goals.

How long is the POC written for?

The POC is written for a 6-month (or 180 days) time frame.

Why is a signed POC from the therapist and doctor important to have?

Once signed by the doctor and therapist, the Plan of Care acts as a prescription for the recommended therapy service within the allotted date range and shows medical need for that service.

What happens when the POC expires?

A re-evaluation is initiated up to one month prior to the current Plan of Care’s (POC) expiration. Our re-evaluation is conducted during the treatment sessions leading up to the POC expiration date. A new POC will be created to show updated goals and recommendations and include a new 6 month date range.

If the Plan of Care is written for a 6 month time, are we committing to 6 months of therapy when we start?

Our POC is written to allow for 6 months of uninterrupted therapy services; however, a child can be discharged at any time when progress and goals are met. Communication between the therapist and parents is crucial throughout the 6-month period.----Take out this sentence: Why is communication between the therapist and parent crucial? What is involved in the re-evaluation process?

Similar to the evaluation, the re-evaluation will include a parent/guardian interview, standardized testing (if able), clinical observations, and discussion with the therapist and parent/ guardian to update the child’s history, current goals and concerns, improvements observed in the past 6 months, and recommendations moving forward.

Is there a fee for a re-evaluation?

If your child has been a patient receiving therapy at Foundations, our re-evaluation process does not have an additional fee for the families.

The evaluation found that my child needs therapy. How do I start therapy?

After the therapist discusses the evaluation findings with the parent/guardian, our office will be informed of the recommendations. At this time, our office will reach out by phone, email, or text to discuss the next step.

How do therapy schedules work at Foundations?

Therapy schedules are set on a consistent basis each week. This provides continuity of care for our patients, families, and therapists.

How can I access my child's therapy information and therapy schedule?

We use a portal system that comes directly to your email, allowing you to access your child’s evaluation, re-evaluation, Plan of Care, home activities, and parent education handouts, as well as check on the upcoming therapy schedule.
