What are hours of operation?

Foundations Therapy is open Monday-Friday between 8:30-6:30 pm

What services do you provide?

We provide physical, occupational, speech and language therapies. We also provide the following specialized services: comprehensive dyslexia evaluations with reading remediation, feeding therapy, and sensory integration therapy.

What age child do you work with at Foundations Therapy?

We provide therapy services for children birth through 21 years old.

How soon can I get my child in for an evaluation? What is your evaluation process?

When you call to to set-up an evaluation, the office staff will complete an intake form with information regarding your child’s personal information, areas of concern, and discipline that is requested. Our office will then request a prescription from your primary care physician that provides the necessary information for insurance companies. Our billing director will check the individual’s insurance plan. It is important to us that we evaluate your child only if and when we have a schedule time slot for therapy, should the evaluation recommend therapy. Our goal is to provide a quality evaluation and consult with the parent regarding the results.

I just had an evaluation completed at another facility, but I need therapy services. Do I need to go through the evaluation process again?

At Foundations Therapy, an evaluation can be used from another facility as long as the Plan of Care has the necessary information for treatment, and it has been completed within the last 3 months.

Are you in-network with insurance companies?

We are currently in network with:

  • Blue Cross Blue Shield
  • United Healthcare (UHC)
  • Tricare
  • CMS/Medicaid Title 19 and Title 21
What if my insurance is out-of-network?

Our billing staff will assist with submitting to insurance providers as an “out-of-network provider.”  In the case that insurance does not cover services, we offer a “private pay discount.”  Please speak to the office for further information.

Do you provide conferences within the community?

We have experience doing conferences on a variety of topics to parents, teachers/educators, and other professionals. Topics we have done include the following: Sensory Processing Disorder, Executive Functioning, Language Literacy, Development, Infant Massage, and Feeding and Oral Motor Development.
